Steps to Playing Competitive

(Steps 2 - 6 are due all at once and need to be turned into the soccer office on or before July 15th)

 

1. Accept your Bid by clicking the above email link and placing in the subject line the player's name, tryout number, and 2011/2012 age group. In the body of the message please indicate whether you have accepted or declined your bid. You have 48 hours to accept your bid.

 

2. Fill out the Letter of Intent (Click on the Green Box Below) - Due July 15th to the soccer office and will be kept by the soccer office

3. Fill out the Medical Release Form (Click on the Green Box Below) - Due July 15th to the soccer office and will be kept by your managers

4. Fill out the Player Code of Conduct Form (Click on the Green Box Below) - Due July 15th to the soccer office and will be kept by the soccer office

5. Fill out and provide payment with the Fees Form (Click on the Green Box Below) - Due July 15th to the soccer office

6. Provide a Copy of the Player's Birth Certificate - Due July 15th to the soccer office

 

7. Fill out and bring with you the Uniform Order Form (Click on the Green Box Below) - Due on your specific team's uniform fitting night which will be communicated by the team manager, fitting nights will take place the week of August 2nd. This will apply to the new U11 teams and any players new to DASC.  Any questions on this can be directed to your team manager during your first team meeting.  Uniform sizing nights will be set up with and communicated to your team manager.
